Pharyngitis,Diagnosis and Empiric antibiotic treatment Considerations

چکیده
Pharyngitis is the common disorder more in children than adults. Pharyngitis is common in the developing countries. In the United States pharyngitis is diagnosed in 11 million patients in emergency department and ambulatory setting annually. Viruses are frequently cause of viral pharyngitis, with adenovirus, rhinovirus, enterovirus, influenza A and B parainfluenza viruses. Streptococcus pyogenes group A(GAS) or group A beta hemolytic streptococcus(GABS) is the bacterial agent of acute pharyngitis. Fusobacterium necrophorum and Mycoplasma pneumonia also been reported. Virulence factors leads to pharyngitis, invasive disease, acute rheumatic fever, and acute glomerulonephritis. Bacterial signs include pharyngea lerythema, tonsillar enlargement, and grayish-whitish exudate covering the posterior pharynx and tonsillar pillars. Symptoms such as conjunctivitis, coryza, oral ulcers, cough and diarrhea suggest a viral cause. Modified Centor score may be used for diagnosis. A 10 days course of penicillin remains treatment of choice, macrolide for penicillin allergic patients, amoxicillin has the advantage of less frequent dosing. Use of broad spectrum antibiotics thought to contribute to antibiotic resistance. Role of tonsillectomy or adenoidectomy in GBHS pharyngitis incidence is poorly understood.

[Diagnosis and treatment of streptococcal pharyngitis].
Common signs and symptoms of streptococcal pharyngitis include sore throat, temperature greater than 100.4 degrees F (38 degrees C), tonsillar exudates, and cervical adenopathy. Cough, coryza, and diarrhea are more common with viral pharyngitis. Available diagnostic tests include throat culture and rapid antigen detection testing. Throat culture is considered the diagnostic standard, although t...

Background: The management of neonatal sepsis especially in developing countries is problematic. There is no single reliable marker of infection available at the present. C-reactive protein (CRP) has long been used as a marker of infection. Serial measurements of CRP are recommended as a guide for duration of antibiotic therapy. Objective: To evaluate the serial CRP measurement as a guideline...

In the present issue of Jornal de Pediatria, Santos & Berezin1 discuss one of the most relevant themes related to general pediatrics: the management of patients with acute pharyngitis. BACKGROUND How to use clinical score, the rapid streptococcal antigen test (RSAT), and culture results is uncertain for efficient management of acute pharyngitis in adults. METHODS This prospective cohort study included 372 adult patients with pharyngitis treated at a Swiss university-based primary care clinic. In eligible patients with 2 to 4 clinical symptoms and signs (temperature >or=38 d...

Background and Objective: Streptococcal pharyngitis is one of the most common bacterial infections that its improper treatment could lead to complications such as heart and kidney problems. Although streptococcal pharyngitis makes up a low percentage of pharyngitis etiology, penicillin derivatives are prescribed for many of the patients with pharyngitis. The aim of this study was to determine t...
